Preschool Details: Connecticut Institute For Communities, Inc. sponsors the Head Start of Northern Fairfield County Program, annually serving more than 350 low-income preschool children and their families through center-based programs and services. Head Start is a Federal program designed to advance the school readiness of low-income children by providing comprehensive early childhood care and education, as well as family development services that include health, education, social services, and parent-community involvement. The Early Head Start program serves lower income infants, toddlers and their families. The program is center-based, and provides early childhood care and development, as well as family support services on a year-round basis.

How to Apply: 1. Program Application 2. Personal Identification (Identification Personal): Driver's License (Ina Licensia) Or/o State Identification (Identificacion del Estado) 3. Proof of Income (Prueba de Ingreso): Tax Forms - 1040 or W2 (Forma de los impuestos - 1040 or W2) Or/o Current Pay Stubs or Letter from Employer (4 Talones de los cheque de pago o una carte del Jefe) Or/o Public Assistance Letter (Una carta de asistencia publica) Or/o Unemployment Compensation (Compensation de Desempleo) 4. Child Support Documentation (if applicable) (Documentacion si recibe ayuda del padre en case de separacion) 5. Child's Birth Certificate: Long Form (Certificado de Nacimiento - forma larga) 6. Immunization Record (Tarjeta de Vacunas) - Authorization for the Administration of Medicine by School Personnel, Refusal of Medication Administration by Head Start Staff (Rehuso De Medicina) 7. Physical (Examen Fisico) - Need TB, HGB/HCT, & Lead Results recorded on form by Physician 8. Proof of Residency (Prueba de donde vive): Utility Bill (Page de Luz), Or/o Lease Agreement (Contrato de Renta) 9. Emergency Contact Information (3 or 4 Numeros de emergencia de familiars o amigos que puedan recojer a su nino (a) en casa de una emergencia y usted no sea localizado)
